RESUMO

Ferroptosis is a novel iron-dependent mode of programmed cell death characterized by iron deposition and accumulation of lipid peroxidation. More and more studies have found that ferroptosis is closely related to the pathogenesis of type 2 diabetes mellitus (T2DM). The yin-fire theory is an important part of LI Gao's spleen-stomach theory, and it is believed that qi-fire imblance and yin-fire internal generation is the main pathogenesis of T2DM. Abnormal iron metabolism may be an important prerequisite for T2DM yin-fire internal generation, while oxidative stress is the specific manifestation of T2DM qi-fire imbalance. Reactive oxygen species (ROS) is the end product of qi-fire imbalance, and lipid peroxide is the pathological products of T2DM yin-fire internal generation. This study intends to explore the pathological mechanism of qi-fire imbalance and yin-fire internal generation from the perspectives of iron metabolism, oxidative stress and lipid peroxidation, enriching the modern connotation of yin-fire theory, and benefiting traditional Chinese medicine to target against ferroptosis, and prevent and treat T2DM precisely.

RESUMO

For the treatment of postherpetic neuralgia, drugs have always played a major but unsatisfactory role. As auxiliary or alternative therapies for postherpetic neuralgia, non-pharmacological interventions, such as electrical stimulation and repetitive transcranial magnetic stimulation, not only have shown favorable efficacy, but also can decrease adverse reactions to drugs with high safety and patient acceptance, and are benificial for management of patients with postherpetic neuralgia.

Objective@#To understand the status quo and influencing factors of overweight and obesity in preschool children, and to provide scientific theoretical reference for the prevention and control of overweight and obesity in preschool children in Urumqi.@*Methods@#Stratified cluster sampling method was adopted to select 1 897 preschool children from 10 kindergartens in Urumqi from October to December in 2021 to understand the status quo of overweight and obesity of preschool children by measuring their height and weight. The influencing factors were collected by questionnaire survey, including Chinese preschoolers eating behavior questionnaire, Chinese preschooler s caregivers feeding behavior scale, 3-6 year old children s home nurture environment scale, and characteristics and influencing factors of physical activity among preschool children.@*Results@#The prevalence of overweight and obesity in preschool children was 31.21% (592), including 19.50% (370) overweight and 11.70% (222) obesity.Childhood overweight and obesity detection rates varied significantly by age, sex, child dietary habits, father BMI, maternal BMI, and maternal pre pregnancy BMI ( χ 2=19.63,28.75,9.45,18.21,18.45,19.36, P <0.05). Multivariate Logistic regression analysis showed that gender, children s eating habits, paternal BMI, pregnancy BMI, satiety responsiveness, external eating, initiative eating, weight concerns, behavior restricted feeding, physical activity and family physical activity environment were the influencing factors of overweight and obesity in preschool children( OR =0.52,1.43,1.51,1.44,0.69,0.74,1.35,1.71,0.81,0.96,1.10, P < 0.05 ).@*Conclusion@#Overweight and obesity in preschool children are popular in Urumqi. Education, diet control and scientific exercise should be strengthened to prevent childhood overweight and obesity.

Corona virus disease 2019 (COVID-19) patients with trauma are at high risk of venous thromboembolism (VTE), which must be taken seriously in the therapeutic processes. Hypercoagulable state is induced by 2019 novel coronavirus (2019-nCoV) in many ways, such as increasing the level of inflammatory factors and fibrinogen, and inducing endothelial cell injury. The venous wall injuries from trauma and operation directly or indirectly trigger off the exogenous coagulation pathway and the microcirculation can be damaged at the same time, which may initiate the exogenous pathway of VTE. Immobilization of limbs and forced bed rest during the treatment of traumatic patients will slow venous blood flow. Chronic non-communicable diseases such as diabetes in the elderly were independent risk factors for VTE. Furthermore, the persistent fever, severe lung disease, respiratory failure, sepsis and invasive technology application add the risk of VTE and the difficulty of treatment. In order to help effective prevention VTE of for COVID-19 patients with trauma, the authors put forward relevant technical suggestions for prevention and nursing of VTE to provide basis for nursing work during pandemic of COVID-19.

Objective@#To analyze the effect of detail nursing in enteral nutrition in patients with severe craniocerebral injury.@*Methods@#A total of 96 patients with severe craniocerebral injury admitted to ICU department were divided into 2 groups of 48 cases according to the Stochastic numerical table method. Both groups were treated with enteral nutrition. During the treatment, the control group was given routine care, and the observation group was given detailed nursing, and the two groups were compared.@*Results@#The incidence of abdominal distension, constipation and diarrhea in the observation group was 4.17% (2/48) and 2.08% (1/48) lower than 16.67% (8/48) and 16.67% (8/48) in the control group, the difference was statistically significant (χ2=4.019, 4.414, P<0.05). The Glasgow Coma Scale (GCS) score was 9.45±1.42, which was significantly higher than 7.19±1.36 of the control group (t=7.963, P<0.05). Compared with the control group, the serum total protein (62.67±3.69) g/L, serum albumin (35.56±2.08) g/L, and peripheral lymphocyte count (1.68±0.15)×109 were significantly higher than (59.15±3.55) g/L, (31.62±2.17) g/L, (1.49±0.26)×109 in the control group (t=4.763, 9.081, 4.385, P<0.05).@*Conclusion@#During the period of enteral nutrition therapy for patients with severe craniocerebral injury, detailed nursing is helpful to reduce the related complications, improve the degree of illness, and improve the nutritional status of the body.

OBJECTIVE: To investigate the effects of Buyang huanwu decoction on the expression of MMPs and TIMPs in cardiac tissue of viral myocarditis (VMC) model mice. METHODS: Male BALb/c mice were randomly divided into control group, model group, positive control group [captopril, 100 mg/(kg·d)], Buyang huanwu decoction low-dose, medium-dose and high-dose groups [6, 18, 36 g/(kg·d)], with 24 mice in each group. Except for control group, other groups were given Coxsackie virus B3 once intraperitoneally to induce VMC model. After modeling, control group and model group were given constant volume of normal saline intragastrically; administration groups were given relevant medicine intragastrically; once a day, for consecutive 30 days. The general situation of mice in each group was observed. The day of inoculation was set at 0 d, heart mass to body mass ratio (HW/BW) was measured at 4, 10, 20, 30 d after inoculation. The morphological characteristics of myocardium were observed by HE staining, and the myocardial histopathological scores of myocardium were evaluated. The distribution of type Ⅰ and Ⅲ collagen in myocardium was observed by Abcam picrosirius red staining, and the ratio of type Ⅰ to Ⅲ collagen was calculated. At 30 d, relative expressions of MMP-1, MMP-3, MMP-9 and TIMP-1 in cardiac tissue were detected by Western blotting assay, and the ratio of MMPs to TIMPs was calculated. RESULTS: Compared with control group, mice in model group suffered from irritability, arch back, alleviation of stimulation response, reduction of body mass and even mental depression. Typical inflammatory changes and local interstitial hyperemia were observed in the myocardium, accompanied by a large number of lymphocyte infiltration and distribution of type Ⅰ and Ⅲ collagen. HW/BW (at different time points of 10-30 d), myocardial histopathological score (at different time points of 4-30 d), ratio of type Ⅰ and Ⅲ collagen (at different time points of 4-30 d), the expression of MMP-1 and MMP-9, ratio of MMPs to TIMPs were increased significantly, while the expression of MMP-3 and TIMP-1 were decreased significantly (P<0.05). Compared with model group, above symptoms of mice in administration groups were improved to different extents. HW/BW [at different time points of 10-30 d in administration groups (except for 10 d in Buyang huanwu decoction low-dose group)], myocardial histopathological score (at different time points of 10-30 d in administration groups), ratio of type Ⅰand Ⅲ collagen (at different time points of 4-10 d in positive control group and Buyang huanwu decoction high-dose group, at different time points of 20-30 d in Buyang huanwu decoction low-dose and medium-dose groups), the expression of MMP-1 (positive control group and Buyang huanwu decoction high-dose group) and MMP-9 (administration groups), ratio of MMPs to TIMPs (administration groups) were decreased significantly, while the expression of MMP-3 (positive control group, Buyang huanwu decoction low-dose and high-dose groups) and TIMP-1 (administration groups) were increased significantly (P<0.05). CONCLUSIONS: Buyang huanwu decoction can inhibit myocardial fibrosis of VMC model mice by inhibiting myocardial collagen hyperplasia, regulating the expression of MMPs and TIMPs, improving MMPs/TIMPs imbalance.

Objective To observe whether transcutaneous electrical acupoint stimulation (TEAS) could improve the emergence and recovery of patients undergoing robotic gynecologic surgery,and to explore the mechanism behind it.Methods Patients (aged 18-65 years,BMI 18-30 kg/m2,ASA grade Ⅰ or Ⅱ) scheduled for elective robotic gynecologic surgery were screened and randomized into three groups:group TEAS (groups T),no acupoint group (group N) and control group (group C),receiving TEAS (ST-36,SP6,BL59,BL60),stimulation at bilateral hips and no-stimulation respectively.Stimulations were given from 30 min before anesthesia induction to the end of surgery.Recovery measurements during emergence,PACU stay and 24 h after surgery were recorded.Levels of serum AQP4,MMP9 and S100β were analyzed.Results Time to response to verbal command and time to extubation in group T [(18.3± 6.7) min and (19.4 ± 6.6) min respectively] were significantly shorter than those in group C [(21.9±7.3) min and (23.1±7.3) min respectively] (P <0.05).Maximum VAS scores during PACU stay were significantly lower in group T than that in groups C and N (P<0.05).Postoperative AQP4 level in group T significantly decreased compared with baseline (P<0.05).However,postoperative MMP9 and S100β level in group C significantly in creased compared with the baseline (P<0.05 or P<0.01).Conclusion TEAS could fasten emergence of patients after robotic gynecologic surgery and improve postoperative analgesia.Mechanisms involving AQP4,MMP9 and S100β may be involved.

From January 2015 to June 2016,107 patients with varicocele underwent microsurgical varicocelectomy in the First Affiliated Hospital of Xiamen University.The clinical outcomes of microsurgical varicocelectomy with(n=56)or without(n=51)microvascular Doppler ultrasonography were compared. All patients were treated successfully by both methods and no postoperative complications such as testicular atrophy and/or hydrocele were found during 12 months postoperative follow-up.For the group without microvascular Doppler,accidental artery ligation occurred in 2 cases and the internal spermatic vessels were unidentifiable in 4 cases, and recurrence occurred in one case.By contrast, no artery injury or ligation occurred during the operation with microvascular Doppler, and no recurrence occurred postoperatively. Microvascular Doppler ultrasonography, which is reliable for discerning arteries and veins, can effectively and safely assist microsurgical varicocelectomy for treatment of varicocele.

Purpose To evaluated the effect of ureteral stent placement before flexible ureteroscopic lithotripsy(FURL).Methods A systematic search of PubMed,Cochrane Library,Embase,Scopus,VIP,CNKI,Wanfang database from databases establishment to February 2017 was performed to identify all clinical trials on the effect of ureteral stenting before flexible ureteroscopic lithotripsy.The outcomes included stone-free rate,mean operative time,success rate of ureteral access sheath placement and postoperative complications.RevMan 5.3 software was used to complete the Meta statistical analysis.Results Three randomized and four non-randomized studies were analyzed,which consisted of 1 176 patients including 788 cases in experimental group,388 cases in control group.Meta-analysis showed significant differences between experimental group and control group in stone-free rate (OR =1.88,95% CI 1.30-2.71,P < 0.001).There was no statistically significant difference in mean operative time between experimental group and control group (WMD =-0.99,95 % CI-10.63-8.65,P =0.84).The success rate of ureteral access sheath placement was significantly higher in experimental group than that in the control group (OR =8.24,95% CI 3.17-21.45,P < 0.001).In term of postoperative complications,two groups had significant differences (OR =0.57,95 % CI 0.33-0.99,P =0.04).Conclusions Preoperative ureteral stenting can increase the stone-free rate and the success rate of ureteral access sheath placement,and reduce complications of FURL.There is no statistically significant difference in mean operative time.

Objective@#To evaluate the reliability and validity of warning signs checklist developed by the National Health and Family Planning Commission of the People′s Republic of China (NHFPC), so as to determine the screening effectiveness of warning signs on developmental problems of early childhood.@*Method@#Stratified random sampling method was used to assess the reliability and validity of checklist of warning sign and 2 110 children 0 to 6 years of age(1 513 low-risk subjects and 597 high-risk subjects) were recruited from 11 provinces of China. The reliability evaluation for the warning signs included the test-retest reliability and interrater reliability. With the use of Age and Stage Questionnaire (ASQ) and Gesell Development Diagnosis Scale (GESELL) as the criterion scales, criterion validity was assessed by determining the correlation and consistency between the screening results of warning signs and the criterion scales.@*Result@#In terms of the warning signs, the screening positive rates at different ages ranged from 10.8%(21/141) to 26.2%(51/137). The median (interquartile) testing time for each subject was 1(0.6) minute. Both the test-retest reliability and interrater reliability of warning signs reached 0.7 or above, indicating that the stability was good. In terms of validity assessment, there was remarkable consistency between ASQ and warning signs, with the Kappa value of 0.63. With the use of GESELL as criterion, it was determined that the sensitivity of warning signs in children with suspected developmental delay was 82.2%, and the specificity was 77.7%. The overall Youden index was 0.6.@*Conclusion@#The reliability and validity of warning signs checklist for screening early childhood developmental problems have met the basic requirements of psychological screening scales, with the characteristics of short testing time and easy operation. Thus, this warning signs checklist can be used for screening psychological and behavioral problems of early childhood, especially in community settings.

Objective To explored the therapeutic benefit and safety of acute kidney injury ( AKI) mice induced by ischemia/reperfusion ( I/R ) treating by thermosensitive chitosan chloride ( CSCI ) hydrogel, an injectable scaffold for adipose-derived MSCs ( ADMSCs ) delivery.Methods After establishing the rat model of acute kidney injury, all rats were randomly divided into 4 groups: ADMSCs/PBS group, ADMSCs/CSCI group, PBS group and CSCI group.Live/Dead staining was used to evaluate the Cytocompatibility between CSCI hydrogel and ADMSCs, dihydroethidium ( DHE) staining was used to detect the number of ROS in vivo, and bioluminescence imaging ( BLI) was used to track the retention and survival of ( fluc-mrfp) labeled ADMSCs in the rat kidney, respectively.The biocompatibility and biodegradability of CSCI hydrogel in kidney tissue were detected by HE staining.The improvement of renal function was evaluated by detecting the level of serum creatinine and urea nitrogen.Results Live/Dead staining manifested an satisfactory Cytocompatibility between CSCI hydrogel and ADMSCs.CSCI hydrogel can improve the micro-environment of AKI kidney tissue by lowering the level of ROS.In PBS and CSCI groups, the positive rate of DHE staining was 68.8%±8.5% and 38.5%±5.8%( P <0.05 ) , respectively.These suggested that CSCI hydrogels could improve the retention and survival of grafted ADMSCs.In ADMSCs /CSCI hydrogel group, BLI fluorescent signal can be detected on the seventh day and was undetectable on the 21st day.ROI value of BLI signal changed from 38 ×105 p/(s? cm2? sr) on the first day to 8.5 ×105 p/(s? cm2? sr) on the 14th day.In ADMSCs /PBS group, BLI fluorescent signal only can be detected on the seventh day and was undetectable on the 14th day.ROI value of BLI signal changed from 17 ×105 p/( s?cm2? sr) on the first day to 3.3 ×105 p/( s? cm2? sr) on the 14th day.Results detected on the 3th and 28th day suggested that ADMSCs/CSCI hydrogel group , ADMSCs/PBS group and CSCI hydrogel group have a better self-repairing capability than the PBS injection group ( P<0.05 ) .In addition, compared with the ADMSCs /PBS group and CSCI hydrogel group, ADMSCs /CSCI hydrogel group got a lower tissue injury scores (P<0.05).At the 4th week, significant improvement of the renal function was observed in CSCI hydrogels with ADMSCs groups.Conclusions CSCI hydrogel is a very promising cell carrier for the treatment of AKI.

Objective To analyze the mental health status and personality characteristic of hyperplasia of breast,so as to provide sound scientific basis for enhancing the quality of life of those patients.Methods Assessments were made by using Symptoms Checklist 90 (SCL-90)and 16 Personality Factor Questionnaire (16-PF) for the mammary gland hyperplasia patients and the normal,and data were analyzed with SAS 9.2.Results (1)Significant differences were found in the scores of somatization,compulsion,sensitivity,depression,terror and paranoid between the byperplasia of breast and heahhy subjects.(2)Multivariate and unconditional Logistic regression analysis revealed that the protective factor of psychosomatic symptoms is the warmth stability and perfectionism which factor regression coefficient was negative; the risk factor of psychosomatic symptoms was the dominance,suspicion,apprehension and tension which factor regression coefficient was negative.Conclusions There are various psychological problems which affected by personality characteristic in the patients with breast hyperplasia,treatment should be optimized based on the personality characteristics of patients with breast hyperplasia,including changing negative attribution model,disseminating preventive knowledge,and finally improve the mental health condition of patients with breast hyperplasia.

Objective To detect whether adipose-derived mesenchymal stem cells (ADMCSs)can differentiate into renal tubular epithelial cells and be used for treatment of acute kidney injury (AKI).Methods ADMSCs were separated and cultured in vitro,and were transfected with lentivirus carrying reporter genes of firefly luciferase and monomeric red fluorescent protein (fluc-mrfp).Transfected ADMCSs (2 × l06) were directly injected into renal parenchyma after establishment of AKI models induced by ischemic reperfusion injury.Distribution and differentiation of ADMSCs in rats were detected by bioluminescence imaging (BLI) and immunofluorescent staining,respectively.Results ADMSCs had positive expressions of CD29 and CD90,rather than expressions of CD34,CD45 and CD31.Lentivirus-transduced ADMSCs had stable expressions of mrfp and fluc reporter genes.BLI showed persistent fluorescence signal in vivo even at day 14 after ADMSCs transplantation.Immunofluorescence staining further indicated that the transplanted ADMSCs could differentiate into tubular epithelial cells.Conclusion ADMSCs in vivo can differentiate into renal tubular epithelial cells,which provides an experimental basis for more researches on ADMSCs transplantation for treatment of AKI.

OBJECTIVE@#To evaluate the effect of chronic nicotine on contextual fear memory in rats.@*METHODS@#Rats were subcutaneously injected saline or nicotine for 14 days continuously to induce nicotine dependence. Somatic signs of spontaneous nicotine withdrawal were assessed on day 1 and 14 after the last injection. Locomotor activity was tested 13 days after the last injection. Contextual fear conditioning task was run 14 days after the last injection, which included 3 phases: habituation, fear conditioning, and test. Percent freezing was used as an index for fear memory during the conditioning and test phases. Finally, footshock sensitivity test was conducted.@*RESULTS@#Compared with the saline group, the global withdrawal scores significantly increased in the nicotine group when the somatic signs were assessed 1 day after the nicotine injection, while there was almost no difference in the 2 groups 14 days after the last injection. In the contextual task, the nicotine group showed a significant increase in the level of freezing response during the test phase but not in the conditioning phase. There was no significant difference in the locomotor activity and footshock sensitivity between the 2 groups.@*CONCLUSION@#Chronic nicotine enhances the contextual fear memory in rats. This effect is not attributed to the change of somatic signs, locomotor activity or footshock sensitivity.

Objective To improve the rheumatologists' understanding of noncirrhotic portal hypertension.MethodsA case of systemic sclerosis complicated by noncirrhotic portal hypertension was reported and the related literatures were reviewed.Results A 51-year-old female who had been diagnosed as systemic sclerosis presented clinically with an chronic onset of portal hypertension.She also had pancytopenia,splenomegaly,and significant esophageal varices.Liver function tests were normal.Hepatitis viral serology was negative.Ultrasound scan of liver revealed no focal lesion.ACT scan confirmed the absence of portal vein thrombosis.Taking into account the above evaluation we concluded that the patient had systemic sclerosis and noncirrhotic portal hypertension.The patient was on prednisolone and immunosuppressant and the condition was well.Conclusion Noncirrhotic portal hypertension complicated by autoimmune disease,especially SSC,is very poor,characteriged by significant portal hypertension as well as histological evidence that cirrhosis is absent.Rheumatilogist should pay attention to it.

RESUMO

Objective To investigate the clinical predictive value of rheumatoid factor (RF), anti-keratin antibodies (AKA), anti-cyclic citrullinated peptide (anti-CCP) antibody and anti-perinuclear factor (APF) in the transition from undifferentiated arthritis (UA) to rheumatoid arthritis (RA), and to analyze the clinical relevant factors. Methods 271 patients with UA who were followed up for a year were enrolled into the investigation. RF was measured by the rate scatting immunity method. APF and AKA were detected by immuniofluorescence method(IFA). Anti-CCP was measured by ELISA. Erythrocyte sedimentation rate (ESR) was analyzed by Wilcoxon method. The duration of morning stiffness, the numbers of swelling and tender joints, tender joints involved and DAS28 score were recorded and analyzed. Results 99% patients who had four-antibody-positive finally convetted from UA to RA. The conversation rate for those who had two or more than two antibody-positive was 83.0% and 65.9% respectively. The sensitivity and specificity of RF and anti-CCP antibody-positive in those who converted from UA to RA was 77.8% and 80.5% respectively. The percentage of polyarticular swelling in antibody-negative, one-antibody-positive, two-antibody-positive, three-antibody- positive and four-antibody-positive was 48%, 57%, 59%, 70% and 70% respectively. Meanwhile, the percentage of multi-small-joint involvement was 71%, 71%, 72%, 76% and 83% respectively. The proportion of elbow involvement in antibody-negative patients was 72%, which was the highest among all joint area involvement. The conversion differences of the group with more than 3 swelling joints or more than 3 small joints involvement ranked the first and second in frequency. Conclusion The combined detection of these autoantibodies could increase the specificity of early diagnosis of RA. The more positive antibodies present, the more likely the concersion form UA to RA. The sensitivity and specificity of RF and anti-CCP-positive is high, so the latter is expected to become one of the diagnostic criteria. The polyarticular swelling and multi-small-joint involvement are valuable in predicting the transformation from UA to RA.

RESUMO

Objective To observe the change of glucocxnticoid(GC) and the messenger RNA expression of its receptor(GRmRNA) in MRL/lpr mice with the intervention of nourishing yin and clearing heat decoction.Methods Forty MRL/lpr mice were randomly divided into four groups:model control group,TCM group,west medicine group,combined treatment of traditional chinese medicine and west medicine group,10 mice in each group.The group was intragastrically administrated normal sodium,nourishing yin and clearing heat decoction,prednisone suspension,nourishing yin and clearing heat decoction plus prednisone suspension,0.5 mL every time,once daily for 10 weeks respectively.Meanwhile,10 Kungming mice were selected as normal group,administrated normal sodium,like the model group.The expression of glucocxnticoid receptor-alpha and beta(GR?mRNA,GR?mRNA) were detected by reverse transcription polymerase chain reaction(RT-PCR).Results Compared with normal,the expression of GRmRNA was increased in model groups,but the level of GC was not different.Compared with model group,GR?mRNA and GR?mRNA of MRL/lpr mice were improved in all treatment groups,especially in combined treatment of traditional Chinese medicine and western medicine group.Conclusion Nourishing yin and clearing heat decoction has effect of upregulating the expression of GRmRNA in MRL/lpr mice,so as to enhance the effect of glucocorticoid and reduce its side effects.
